- **Root cause measured:** three identity shifts today (`b314b93`, `e10ce42`, `9981faf`) copied repo → dev → prod directly, seconds apart — never through the tool. Content was fine (`repo == dev == prod`, 16/16 shas, `source-sync-check` rc 0 both envs); only provenance was missing.
- **Own reviewer gate:** `REVIEW-20261013T084024Z-deploy-path-ledger.md` → **`VERDICT: APPROVE`**, reviewer re-ran all 10 suites (**1,269 passed / 0 failed**) and measured contrast (2.6:1 → **5.68:1 / 6.30:1**).
