### Changed — one new rule in one tool, its suite's fixtures, and the two compared exit-1 copies

- **R8, the rule the other seven structurally cannot be.** R1–R4, R6 and R7 judge `CHANGELOG.md`; R5 judges `agent-logs/PROGRESS.md`; each file can be perfectly healthy while the two disagree about the same run — and that is not hypothetical: `[0.4.160]`'s entry was committed with its own `PROGRESS.md` append **staged out of the tree** (`git checkout -- agent-logs/PROGRESS.md`, to keep the monitor suite's clean-tree assertions green during a 19-minute regression) and never re-appended, so for ≈23 minutes `grep -c "0.4.160" agent-logs/PROGRESS.md` read **0** — the authoritative file still told the next run to do an item the changelog had already struck — while this tool exited **0** with `violations []` the whole time, and `queue-source-check` exited 0 the whole time, because no rule asked whether the two files were *current with each other*. R8 asks only that: the newest `## [x.y.z]` entry's version token must appear in `PROGRESS.md`, reported as `newest_version_in_progress`, and it is the **sixth** cause in the exit-1 list.
- **The reader is `version_named_outside_fences()`, a separate walk on purpose.** It shares `count_queue_bullets()`'s **fence rule** (a version quoted in a pasted block is not a run recording itself — the same way this file quotes a `### Queue` section without gaining one) and adds **digit/dot boundaries** in both directions: `0.4.16` is not satisfied by text reading `0.4.160`, and `10.4.160` does not answer for `0.4.160`, so both spellings a run actually writes — `[0.4.161]` and `0.4.161` — are in scope without a bracket requirement (a formatting rule wearing a defect's costume). It is judged **only when a newest version exists**: with no `## [x.y.z]` heading the newest-entry rules already refuse the file, and naming "the newest entry" of a file that has none would be inventing one. It is deliberately a *third* reader of this file rather than a clause bolted onto R5: they ask different questions (entries that open a queue vs. a version that appears), and a shared walk would make them agree by construction — the reason the census is separate from `parse()`.
- **Known false positive, stated in the docstring and in REGISTRY rather than hidden**: another identity may own the newest entry and keep its record where this rule does not read, which reddens for a green reason. The cure is the behaviour the rule enforces — changelog entry and the run's own `PROGRESS` entry in **one commit**, which every main-loop run already does — and a red here costs a sentence of prose, while a green it cannot see costs a stale queue.
- **Fixtures, so that a red can only ever be R8's.** `make_repo`'s `PROGRESS.md` heading now names the newest version (`## 2026-09-26T00:00Z run - [0.4.67] written here`), which is the isolation discipline section D states for the freeze applied to the second file: without it every builder case — B, D, E, F, J1, M1–M4, M7, M11, M12 — would go red for a rule no case is about, and `M4`/`M11`'s mutants would read *evaded*. Three hand-written fixtures that rewrite `PROGRESS.md` whole (section **L**'s three shapes, and mutants **M9**/**M10**'s two) gained the same heading for the same reason — **M9/M10 need it or their mutants never reach 0**, which is what makes those two mutants still mean what they meant. The assertions that pin the JSON contract follow the key count: `A20` and `H16` are relabelled **11 keys → 12** (`newest_version_in_progress` joins `rules`; the top-level object stays **11**, pinned by `A19`/`H5`), and `O2` now pins the declaration as `A1 A2 A3 R1–R8`.
- **The two compared exit-1 copies were reworded together**: epilog and REGISTRY's ``- `1` - `` bullet both now end `… , a ### Queue heading was not read as a section, or the newest CHANGELOG entry is not named in agent-logs/PROGRESS.md`, i.e. **6 causes** from **5**, still word-for-word identical (section **P**'s `P6`) and still ≥5 on both sides (`P7`). `system-status`'s two deleted copies are untouched — it still only points at `violations[]`.

### Verified — measured on this tree, before and after

- **The suite: `bash tests/test_queue_source.sh` → 209 passed / 0 failed**, exit 0 — **181 → 209**, i.e. **+28 = Q's 24 + M13's 4**, with section tallies **A 21, B 8, C 6, D 6, E 6, F 5, G 14, H 16, I 6, K 6, L 9, N 23, J 7, M 26, O 12, P 14, Q 24** (sums to 209) and no assertion removed or loosened.
- **Pre-fix replay, the control that makes the new numbers mean something**: the pre-step tool (`git show HEAD:tools/queue-source-check`, md5 `bed2c1eb…`) plus the pre-step `system-status` and REGISTRY, run through *this* suite from `/tmp/opencode/q109/pre/` → **178 passed / 31 failed**, exit 1, first red `A20 rules key set is exactly the documented 12 keys`. The 31 reds are the whole step: `A20`, `H16`, `O1`/`O2` (the declaration), `O3`/`O5`/`O6`/`O9` (their preconditions, because the old docstring declares no `R8`), `P6`/`P7`/`P14` (the cause lists), `M13`'s four, and **Q2, Q4–Q6, Q10–Q11, Q13–Q21, Q23**. `Q4`/`M13a` are the defect itself reproduced: **the old tool exits 0** on a `PROGRESS.md` that never names the newest entry. `Q1`, `Q3`, `Q7–Q9`, `Q12`, `Q22` and `Q24` are green on the old tool by design — they pin the healthy shape, the untouched rules and the bare spelling, which is what identifies them as guards rather than findings.
- **Live tree**: `./tools/queue-source-check` → **exit 0**, `one queue: [0.4.160] pointer-only, 111 item line(s) frozen across 116 section(s), 118 PROGRESS bullet(s)`, `rules.newest_version_in_progress: true`; `--format json --nonsense` → exit 2 with the same **12** `rules` keys (the usage path's are `null`, only `frozen_items_expected` stated); `--help`'s exit-1 block and REGISTRY's bullet both enumerate the **6** causes.

### Notes

- **One visible step per run, and this is it**: no `app/src/php` edit → **no reviewer gate and no promote** (prod untouched); **no file under `tests/` or `tools/` other than `tools/queue-source-check`, `tools/REGISTRY.md` and `tests/test_queue_source.sh` was written**, so the tool census is unchanged (**22 tools, 32 registered sections**) and the `- Live:` suite figure on `## regression-run` is untouched (**76, measured 2026-10-01**) — no suite was added, so `tests/test_registry_coverage.sh` section F has nothing to re-read. No new tool, no tool option, no new JSON key beyond the one `rules` key this entry documents, no `GLADEX_APP_VERSION` bump (stays **0.4.28**); no service restarted; **zero DNS writes**; **no mail sent**; model spend **0.00** (free `*-free` models only). Fixture and mutant copies live under the suite's own `$SB/` (trap-removed) and `/tmp/opencode/q109/` only.
