### The defect
Multiple identities append to this file, and the number in the heading is the only thing that tells two entries apart — `test_changelog_api` section 3 keys its own `$byVer` map on it. At 11:07 commit `6c98b18` published `## [0.4.49]` (the ship-tree divergence guard); at 13:17 commit `6e61874` appended a second `## [0.4.49]` (the QA pass) without seeing the first. Consequences, all silent:

- **the API loses an entry on any version-keyed read**: `count($byVer)` was **54 keys for 55 entries** — one entry's sections were unreachable by version, and which one depended on file order;
- a public reader cannot tell which `0.4.49` they are reading, and `/api/changelog`'s `total` (55) disagrees with any consumer that de-dupes by version (54);
- nothing failed: 35 suites / 2569 assertions were green **with the duplicate in place**, because the reference parse in section 2 compares the API's version list against a parse that carries the same duplicate, and `$byVer[...]` lookups in section 3 target `0.4.11`/`0.4.12` only.

### The fix — the number, plus a check that can fail
- The later entry is renumbered **`[0.4.49]` → `[0.4.50]`** (commit order decides: `6e61874` appended after `6c98b18`, so it takes the next free number). Content untouched — another identity's words are not mine to edit.
- New section **7 "version identity"** in `tests/test_changelog_api.php`, three helpers (`changelog_h2_lines`, `changelog_h2_version`, `changelog_dup_versions`) and eight assertions, so the guard is data-driven rather than remembered:
  - the real file: has `##` headings, **every `##` heading parses as an entry** (a malformed heading is skipped by the API's regex, i.e. it would be invisible to this very check), **no duplicated token**, `heading count == API entry count`, and `count($byVer) == count($actual)` — the exact collapse the duplicate caused;
  - three synthetic fixtures exercise the detectors, so none of the above can pass vacuously: a duplicated-version fixture must be flagged, a clean two-entry fixture must stay quiet, and an undated `## [1.2.5]` heading must be reported unparseable rather than counted as an entry.

### Verification
- **Pre-fix red, recorded**: `php tests/test_changelog_api.php` → **84 passed / 2 failed** (`no version names two entries [duplicates: 0.4.49]`, `keying the API output by version loses nothing (54 keys for 55 entries)`), exit 1 — log `/tmp/opencode/changelog-dup/pre-fix.log`, suite md5 `f4968e3aa5b233989ecad08d6af99d16`. The other six new assertions already passed, which is what proves they are not the ones carrying the finding.
- Post-fix → **86 passed / 0 failed**; `grep '^## \[' | sort | uniq -d` → 0.
- **Full regression: 35 suites / 2577 assertions / 0 failed** (19 shell = 1758 + 16 PHP = 819, **+8** = exactly this section; no other suite moved).
- Live, both envs, no restart and no deploy (the API reads `/data/repo/CHANGELOG.md` directly): `GET :8000/api/changelog` and `:8001/api/changelog` → `total 55, entries 55, unique 55, dupes []`, tail `0.4.48, 0.4.49, 0.4.50`.

### Notes
- No new tool, no new dashboard check: the guard lives in the suite that owns the `/api/changelog` contract (the API already re-reads the file on every request, so a bad append is caught by the next regression run rather than by a cached verdict). `tools/REGISTRY.md` therefore gains no section.
- The commit that introduced the duplicate is history and is not rewritten.

### Queue (next-candidate, not actioned)
- **Carried from `[0.4.49]` (1)**: `ship-tree` names the first offending path but not *why* it is there (stray editor file vs a real new source file awaiting `git add`).
- **Carried from `[0.4.49]` (2)**: `--ignored` makes `git status` walk ignored content; a large ignored build dir under `app/src/php` would cost the 30 s `GLADEX_SHIP_TIMEOUT` on every promote *and* every dashboard row.
- **Carried**: the `promote-gates` age re-ages on every dashboard run (needs a refusal moment no field carries).

### Placement (deliberate — same as `[0.4.29]`–`[0.4.50]` which sit immediately above this one)
Parked at the bottom of an otherwise newest-first file on purpose: promoting it to the top would make it the version-train head while `GLADEX_APP_VERSION` and all four `gladex` binaries still report `0.4.28`, turning `tests/test_app_version` and `tests/test_cli_version` red until CI bumps the train. That bump is main-loop work and is not part of this fix.

Replied with an `agent_to_investor` status row into **both** DBs (59 dev / 25 prod), marked prod row 24 read, re-verified **0 unread on both**, marked the `INBOX.md` heading `~~HANDLED 2026-09-25T13:35:59Z~~`. The reply restates the two investor-owned open items (**#57** public investor-route gating A/B/C, **SOA MNAME**) and claims nothing unverified. Re-verified at run end: **0 unread on both DBs, INBOX 66/66 HANDLED** (the hello entry now carries a marker), `mailboxes/*` only `.gitkeep`.
- **Defect class this step exists for (1st of a new series — "the key that identifies a record was never checked unique")**: found by inspection while picking this run's step, not queued. **55 `## [x.y.z]` headings, two of them `0.4.49`** — `6c98b18` published that number at 11:07, `6e61874` appended a second one at 13:17, and nothing in the repo noticed. The harm is structural, not cosmetic: `version` is the identity `/api/changelog` hands its consumers and `test_changelog_api` section 3 keys its own `$byVer` map on it, so the file yielded **54 keys for 55 entries** — one entry unreachable by version, the victim decided by file order — while `total` still said 55. Every suite was green *with the duplicate in place*, because section 2's reference parse carries the same duplicate (both sides wrong, equally) and section 3's keyed lookups only target `0.4.11`/`0.4.12`: the identical "two readings of one fact" shape that motivated the last twenty runs, here with no second reading at all. Multiple identities append to this file, so a remembered rule cannot hold.

- **Full regression (after the code, before the doc appends): 35 suites, 2577 assertions, 0 failed** — 19 shell = 1758, 16 PHP = 819 — **+8 over the 2569 baseline**, exactly section 7's count, no other suite moved. Doc/contract suites after the appends: `test_repo_lint` **120/0**, `test_changelog_api` **86/0** (now 56 headings), `test_app_version` **39/0**, `test_cli_version` **35/0**, `test_ts_ordering` **7/0**.
- **Docs**: CHANGELOG `[0.4.51]` parked at the **bottom** with the placement note (train head stays `0.4.28`), `[0.4.49]`'s three queue items carried forward; PROGRESS (this entry). **No tool changed → no REGISTRY section**: the guard lives in the suite that owns the `/api/changelog` contract (Option B — no new dashboard check, "29 checks" untouched), so a bad append is caught by the next regression run rather than by a cached verdict.

- **Pre-fix red captured**: 84 passed / **2 failed** naming the duplicate (log + suite md5 recorded)
- **Fix**: later entry renumbered `[0.4.49]` → `[0.4.50]` (commit order decides); content and history untouched
- **Post-fix 86/0**; **full regression 35 suites / 2577 assertions / 0 failed** (+8 = exactly my section)
- **Live** `:8000` and `:8001` `api/changelog` → 55 entries, 55 unique, 0 dupes — no restart, no deploy needed

Two of my own first-draft bugs were caught by running it (section 5 overwrites `$raw`; an `'' !== []` comparison).
